In a heat proof bowl, slightly beat the egg yolks and set aside.
Mix the sugar, cocoa powder, cornstarch and salt in a 2-quart sauce pan. Gradually stir in the milk and heavy cream.
Cook over medium heat, stirring constantly, until the mixture thickens and comes to a boil. Continue to boil and stir for 1 minute.
Gradually pour a little of the hot milk mixture into the egg yolks, stirring constantly to temper the eggs. Then add the egg mixture to the saucepan, bring to a boil, stirring for 1 minute.
Remove from the heat, stir in the vanilla, butter, and chopped chocolate until melted and smooth. Pour into dessert dishes and cover the tops of the dishes with plastic wrap, letting it rest on the pudding surface to stop a skin from forming. Once cooled slightly, refrigerate until ready to serve.
